What is insulin resistance? The root cause almost no one explains to you

You are not lazy, and you are not broken. You may simply be metabolically resistant — one of the most changeable things in medicine.

The question I get more than any other is this: "Doctor, what actually is insulin resistance?" You've heard the phrase everywhere — but almost no one explains it in plain words. So let me, because understanding this one idea explains a lot: the weight that won't move, the afternoon energy crash, the numbers your doctor says are creeping up. Insulin resistance is the root under so much of it, and the root is where I like to work.

Insulin is a key

Start simple. Every time you eat — especially carbohydrates — your blood sugar rises. Your pancreas responds by releasing a hormone called insulin. Think of insulin as a key: it unlocks your cells so glucose (sugar) can move out of the blood and into the cell to be used for energy. In a healthy system, a little insulin does the job, blood sugar settles, and you feel steady. That's how it's supposed to work.

Resistance is when the locks get stiff

Now imagine years of too much sugar and refined carbohydrate, too little movement, short sleep, and constant stress. Over time, the cells stop responding well to insulin's knock — the lock gets stiff, and the key doesn't turn easily. That is insulin resistance. Your body's fix is to shout louder: the pancreas pumps out more and more insulin to force the glucose in. For a while it works, and your blood sugar can still look normal — which is exactly why this is so often missed. The high insulin comes quietly, often years before a routine glucose test looks off.

Why it matters

Chronically high insulin isn't harmless background noise. Insulin is also your body's main "store fat, don't burn fat" signal — so when it stays high, losing weight (especially around the middle) becomes an uphill battle, and you can feel it as cravings, fatigue, and brain fog. Left alone, the demand can outpace what the pancreas can supply; blood sugar starts to rise, and that's the road toward prediabetes and type 2 diabetes. This is why I tell people: your weight is often a symptom of what insulin is doing, not the cause. Treat the root and the symptom follows.

The hopeful part

Here's the good news I never want anyone to miss: for most people, insulin resistance is driven largely by daily inputs — so it often responds to daily changes. The body can regain sensitivity. It is, for many, not a life sentence but a signal, and signals can be answered.
